REGISTRATION

  1. Members are required to fill a membership form.

  2. The signing of the form shall be regarded as followed:
    • Signing Head of Department
    • Letter of Appointment.
    • IC for locals (Citizen) / Passport for contract, locum and daily paid (Non-Citizen)

  3. External user of the library are required to bring with them the letter of approval by one of the Senior Officer as below:
    • Director General of Medical Services
    • Hospital Director
    • Chief Executive Officer
    • Head of Library Committee

  4. Registration of members is conducted during office hours only.

  5. Members must notify to the Administrative Head of Medical Library immediately of any changes of address.

  6. Internal and External members of the library must renew their membership annually.

  7. Internal (non-citizen) are required to pay B$200.00 and External members are required to pay B$300.00 for deposit payment, payable to the Account Section, RIPAS Hospital and must bring the receipt to the Administrative Head of Medical Library.

  8. Application for the refund of any money due to members such as library deposit must be made to the Account Section, RIPAS Hospital in writing when membership ceases. Library clearance for such refunds will only be given after the library membership card and tickets has been returned and after any outstanding fines or other changes incurred by damage to or non-return of books have been deducted there from.

  9. Membership of external members will lapse and the deposit (if any) forfeited if membership has not renewed for a period of three consecutive years.
